Insurance
Park CityOur Park City practice is out-of-network with all insurance plans. We do not bill or submit claims on your behalf, and payment is due at the time of service. If your plan offers out-of-network benefits, you may submit your receipt and visit note directly to your insurance company for possible reimbursement.
Heber CityWe are currently completing the credentialing process at our Heber City location and expect to accept all major insurance plans within the next few months. Until credentialing is complete, Heber City is also out-of-network. Please check back for updates, or call us with questions about coverage.
Cancellation Policy
We reserve time for each patient and ask for at least 24 hours’ notice to cancel or reschedule. Cosmetic and procedure appointments require 48 hours’ notice. Late cancellations and missed appointments may require a deposit to book future visits. If you arrive more than 10 minutes late, we may need to reschedule your visit. If an emergency or illness prevents you from keeping your appointment, please call us as soon as you can.
